Give us permission to send your medical records to your new GP
If you change GP in the Netherlands, due to relocation or other reasons, you must give us permission to send your file.
- Download the consent form and fill it out completely.
Don't forget to sign at the end of the form.
- Send the completed and signed form to the practice (the (email) address is on the form).
Let us know who the new GP in the Netherlands will be, and we will send your file there.
If you move abroad, we are required to keep your file for 20 years, unless you explicitly do not want this. You can always request a copy of your medical file.
